2) Valid Passport
3) Valid US Multiple entry visa – or proper
documentation to enter a third country
4) Passport photo (2x2)
5) Visa Fee ($100, or $110 based on type of visa)
– Accepted Cash($100 bills not accepted) /Money order or travelers check in the
name of THE BAHAMAS CONSULATE GENERAL
6) Proof of travel - Itinerary – Original &
Copy
7) If applying by mail – Payment for return postage
by post office next day express mail service or Federal Express etc., must be
sent/submitted
Along with above we need below for applicants holding F, J or H

i. VALID 1-20 FORM SIGNED BY THE INTERNATIONAL
FOREIGN STUDENT ADVISOR.
ii. VALID 1AP-66 FORM SIGNED BY THE INTERNATIONAL
FOREIGN STUDENT ADVISOR.
iii. VALID APPROVAL PETITION FOR H-1 SIGNED BY THE
IMMIGRATION AND NATURALIZATION SERVICES.
